For Referrers
Thank you for referring your patient to Multidisciplinary Sydney Pain. We provide specialist assessment and treatment of chronic and persistent pain, using a multidisciplinary, evidence-based approach, from clinics across Sydney and in Goulburn.
How to refer a patient

Your patient can also book online and upload their referral using our patient booking form. We accept PDF, Word or photo files up to 10 MB.

Helpful information to include
It helps us to process a referral promptly if it includes:
- The patient’s full name, date of birth and a contact number
- The reason for referral, including where the pain is, how long it has been present and any treatment tried so far
- Relevant medical history and current medications
- Recent imaging or other investigation reports
- For a Work Cover or CTP third party claim, the claim details and the initial consultation approval
Patients we see
We see private patients and patients with third party claims (Work Cover/CTP), and we welcome all work cover and third party approvals. We also see patients on a disability pension or an aged pension. Private patients are asked to pay a $50 booking deposit to secure their appointment.
Appointments
An initial consultation takes approximately 40 minutes, and a review consultation approximately 20 minutes.
What we treat
See the chronic pain conditions we treat, including back and neck pain, neuropathic pain, fibromyalgia, headache and pelvic pain. We also perform a range of image-guided interventional pain procedures, such as nerve blocks, epidural steroid injections, radiofrequency treatment and neuromodulation.
