Trigeminal Ganglion Pulsed Radiofrequency and Block

The trigeminal ganglion is a collection of nerve cell bodies that carries sensation from the face. Pulsed radiofrequency treatment may be considered for carefully selected patients with severe facial pain, particularly trigeminal neuralgia, when medication and other treatments have not provided adequate control and a trigeminal ganglion block has demonstrated a useful response. Using X-ray guidance and careful anatomical imaging, a specialised needle is positioned at the target near the trigeminal ganglion. Local anaesthetic may be used to confirm the target, followed by controlled pulsed radiofrequency energy. PRF is intended to modulate pain signalling rather than create the sustained high-temperature lesion used for thermal radiofrequency procedures. The aim is to reduce the frequency or intensity of facial pain while preserving normal nerve function. The response varies considerably between patients, and the treatment does not remove the underlying cause of trigeminal neuralgia.
