Piriformis Muscle Injection

The piriformis is a small muscle deep in the buttock that helps rotate the hip and lies close to the sciatic nerve. Irritation or spasm of this muscle can contribute to buttock pain and, in some patients, symptoms that extend down the leg. A piriformis muscle injection may be considered when clinical assessment suggests that the muscle is an important pain generator.

Using ultrasound or other appropriate image guidance, a fine needle is directed into or around the piriformis muscle and medication is injected. Depending on the clinical situation, this may include local anaesthetic and corticosteroid, with botulinum toxin considered in selected cases. Image guidance helps confirm the location of the muscle and avoid nearby nerves and blood vessels.

The injection may reduce local pain and muscle spasm, creating an opportunity to improve movement and participate more effectively in physiotherapy. The duration of benefit varies and the procedure does not correct every underlying cause of buttock or sciatic-type pain.
