Iliotibial Band Injection

The iliotibial band (ITB) is a strong band of connective tissue running along the outside of the thigh from the hip towards the knee. Irritation of the tissues around the ITB can contribute to pain on the outer side of the hip or knee, particularly with repetitive activity, running or cycling. An injection may be considered when conservative treatment has not adequately settled local inflammation or pain. Using ultrasound guidance, a fine needle is directed to the specific painful structure associated with the ITB rather than injecting indiscriminately into the band itself. Depending on the diagnosis, local anaesthetic and, where appropriate, corticosteroid may be used. The aim is to reduce local pain and inflammation while the underlying biomechanical contributors are addressed. An injection may provide a useful period of symptom relief that allows more effective physiotherapy, stretching, strengthening and modification of aggravating activities. The benefit and duration vary between patients, and repeated injections are not appropriate for every condition.
