Ganglion Impar Block

The ganglion impar is a small collection of sympathetic nerve tissue located at the front of the lower sacrum. It can contribute to persistent pain from the coccyx, perineum, rectum or lower pelvic region. A ganglion impar block may be considered when the clinical pattern suggests a sympathetically mediated component to this type of pain.

Under X-ray guidance, a fine needle is advanced through or around the sacrococcygeal region to the ganglion impar. Contrast is used to confirm the position before a small amount of local anaesthetic, with or without corticosteroid, is injected. The approach is selected according to your anatomy and the precise source and distribution of your pain.

The block may provide temporary pain relief and can help determine whether the ganglion impar is contributing to your symptoms. A clear response that does not last may support consideration of radiofrequency treatment in selected patients.
